Terms for spa memberships:
Membership services available Monday – Friday.
Membership does not include gratuity.
Services must be received within each calendar month or forfeited.
First month dues paid in advance upon enrollment (example: enroll in November 2009, membership begins December 2009).
Subsequent month dues will be charged to the credit card on file the first banking day of the month following enrollment (example: enroll December, 2009, the first bank charge will be February, 2010).
Membership changes or cancellations require 30-day written notice via e-mail to info@caruh.com.
Service cancellations require 24 hour notice via telephone (206-525-4774).
Memberships are 12 months unless renewed by member.
$30 bank fee will be charged to the card on file for NSF and a second bank draft will be attempted.
Membership will be suspended during any month that a charge is not valid after the second draft attempt.
